China market entry for medical devices: an NMPA registration guide
The registration pathway, how NMPA compares with FDA and TGA, and the questions overseas manufacturers ask most often.
The NMPA registration pathway at a glance
- Regulatory strategy. Confirm the registration objective and classification, and choose the clinical evaluation route.
- Product technical requirements (PTR). Draft the PTR, the foundation document that defines specifications, test methods and acceptance criteria.
- Type testing. Safety and performance testing against the PTR, by a qualified testing centre or by the registrant (self-testing).
- Clinical evaluation. Exemption catalogue, same-variety comparison, or clinical trial data. This step usually runs in parallel with type testing.
- Dossier preparation. Technical file, IFU and labelling; notarised English originals with certified Chinese translations.
- Submission and review. CMDE technical review, responses to deficiency letters, administrative approval and the registration certificate.
Steps 3 and 4 usually run in parallel to shorten the overall timeline. End-to-end registration generally takes 1.5 to 3 years for Class II and III devices, depending on classification, clinical route and the number of review rounds.
FDA, TGA and NMPA compared
| US FDA | Australia TGA | China NMPA | |
|---|---|---|---|
| Quality system | QMSR (21 CFR 820, aligned with ISO 13485) | ISO 13485 via conformity assessment | Medical device GMP |
| Safety and performance | Recognised standards and special controls | Essential Principles | Essential Principles of Safety and Performance; PTR |
| Classification | Class I / II / III | Class I / IIa / IIb / III / AIMD | Class I / II / III |
| Primary pathway | 510(k), De Novo, PMA | ARTG inclusion via conformity assessment | NMPA registration (CMDE review) |
| Clinical evidence | 510(k) substantial equivalence or PMA clinical data | Clinical evidence guidelines | Exemption catalogue, comparison, or trial (overseas data accepted) |
| Submission language | English | English | Simplified Chinese (mandatory) |
| Local representative | US Agent | Australian Sponsor | Legal agent in China |
The three systems differ in procedure and language, but they rest on the same evidence logic: benefit-risk assessment, essential safety and performance principles, and a hierarchy of clinical evidence. A dossier that is well argued for one regulator is a good starting point for the others, provided it is restructured for each.
Common questions
Can we just translate our FDA or TGA dossier into Chinese?
No. NMPA expects evidence structured to its own technical requirements. Translation alone often leads to deficiency letters and months of delay.
Will FDA clearance or TGA approval speed up NMPA review?
It helps credibility, but it does not exempt the device from NMPA testing or clinical evaluation. Plan for a full submission, not a fast track.
Do we need a clinical trial in China?
Often not. Many devices use the exemption catalogue or a same-variety comparison, and overseas clinical data can be accepted if it meets NMPA requirements. Choosing the route early saves a year later.
When should we involve a regulatory consultant?
Before design freeze if possible. The cheapest changes are the ones made before testing begins.
How BBM can help
BBM prepares NMPA clinical evaluations, evidence gap analyses and deficiency responses, and translates registration dossiers into Chinese. NMPA regulatory advice is provided with our partner Best-Road (Beijing). See Regulatory Consulting and Translation & Interpretation.
